A: Initial visits at PROVOKE Health are self-pay. The initial consultation fee is $295 for an hour. Follow-up visits are billed at $155 for a half-hour. Insurance often covers labs from Quest or other national labs ordered during your consultation.

As for our South Tampa location, it is exclusively for select appointments with our founder, Dr. Matt Lewis. These appointments are arranged individually on a case-by-case basis.
