Dr. Matthew Lewis, D.C., DACBN, CFMP® is the Founder of PROVOKE Health — a Tampa, Florida-based functional medicine and integrative healthcare practice that is committed to motivating, guiding, and supporting patients on their journey to regaining their confidence, resiliency, and health. By creating and co- managing highly personalized treatment plans that address complex health problems, Dr. Lewis and the team at PROVOKE Health repair patient’s confidence, resiliency, and health.
Dr. Karalynne Blochberger, MD, is the Medical Director for PROVOKE Health in Tampa, Florida, where she practices Functional Medicine and Integrative Healthcare alongside FHG’s founder, Dr. Matthew Lewis. Certified by the American Board of Family Medicine (ABFM) and a member of the American Academy of Family Physicians (AAFP), Dr. Blochberger has practiced medicine at Access Primary Care (Orlando, FL) and Advent Health Primary Care Plus (Orlando).

Cara Palermo, FNP, a valued member of the healthcare team at PROVOKE Health serves as a family nurse practitioner (FNP) specializing in medical assessments and IV infusions for patients grappling with complex and chronic health conditions. With over a decade of hands-on experience in diverse healthcare settings, her focus also includes collaborating with our doctors to identify and address the root causes of our patients’ illnesses.
Alison DeRosa, MBA, is the Chief Operating Officer of PROVOKE Health in Tampa, Fla., where she oversees the implementation of our organization’s short-term tactical trajectory and long-term goals and objectives. She also helps shape and monitor our quarterly and annual budgets, manages our human resources department, oversees our daily operations, and ensures that our healthcare practice remains in compliance with all regulations, guidelines, and standards as required by local, state, and federal entities.
